Lords of the Fallen has some new quests after the release of Patch v.1.1.414 or part 2 of the "Season of Revelry" which rewards you with some new awesome-looking armor sets and weapons for each faction. Lords of the Fallen Herald of the Maw Guide - How to Complete Umbral Quests Fast & Easy

The Lords of the Fallen Herald of the Maw quest for the umbral set is only a few steps long and it revolves around solving a musical puzzle in the Revelation Depths and finally making use of those brazers that you can ignite. This can be done at any point in the game regardless of if you're on a normal run or a new game plus run. Moreover, the LOTF items from the quest are permanent. Because this quest requires a bit of running back and forth through the Revelation Depths, you might find it easier to do after you've cleared out some of the enemies.

LOTF Umbral Quest Walkthrough

Step 1: Visit Umbral Shrine

Starting Point: Begin at the Vestige of Katran.

Route Options:

Via the Sistern: Make your way through the Sistern to reach the Umbral Shrine.
Via the Elevator Shortcut: If unlocked, use this shortcut, but note that you will need to backtrack slightly to reach the shrine.

Shrine Identification: The shrine is distinguishable and there are a few of these throughout the area. Start at the one recommended in your description.

Step 2: Perform Gesture

Gesture: Perform the "Putur Mother's Embrace" gesture in front of the Shrine.

Obtaining the Gesture: If you don’t have this gesture, acquire it by interacting with the Shrine of the Puted Mother in Skyrest Bridge.

Sequence Trigger: Performing the gesture triggers a sequence of three different notes that continually repeats as long as you are in the area.

Attention to Audio: Increase your in-game volume to clearly hear the sequence for the puzzle.

Step 3: First Sequence

Note Sequence: The first sequence consists of three notes - low pitch, medium pitch, then high pitch.

Braziers Interaction:

Each brazier in the area plays one of these three sounds when ignited.
Ignite three braziers in the correct order (low, medium, then high) to match the sequence.

Sequence Consistency: The sequences and braziers are the same for every player.

Step 4: Second Sequence

Return to Shrine: After the first sequence, go back to the shrine to listen to the second sequence.

Note Sequence: This time, it's a medium pitch, followed by another medium pitch, and then a low pitch.

Braziers Interaction: Follow the video or guide to find and ignite the correct braziers in this order.

Step 5: Third Sequence

Final Shrine Visit: Return to the shrine once more for the last sequence of notes.

Note Sequence: The final sequence is high pitch, then low pitch, and finally medium pitch.

Completing the Sequence: Follow the instructions to ignite the braziers in this specific order.

Step 6: Collect Loot from Umbral Belly

Quest Completion Signal: Upon completing the final sequence, triumphant music plays to signal the completion of the quest.
